作為個案管理師，您將是照顧服務中的核心角色，負責評估、計劃並協助服務對象獲得最合適的照顧。
(一)擬定照顧計畫與服務連結
1. 提供社區整體整體照顧體系A級單位服務
2.依照管中心轉介長照需求者服務
3.至個案家與長照需求者或家屬討論,依<個人額度>,照顧問題清單及長照之需求後擬定照顧計畫,照顧計畫送照管中心核定後連結服務或資源
(二)照顧管理
1. 依個案需求時效內完成個案照顧計畫、執行服務計畫及服務品質追蹤。
2. 接受個案及其家屬有關長照服務諮詢、申訴及處理。
3.提供個案之社區資源整合，連結不同專業照護服務資源。
4. 個案管理相關業務之行政作業。
5.追蹤服務計畫/每月追蹤服務品質追蹤
(三)家庭關懷訪視
1. 計畫撰寫服務與執行,核銷成果統計與結報
2.社區長照宣導
3.協助機構行政工作

Adopting Buurtzorg’s onion model of care to create Taiwan’s Persson-Centered Care
TAIWAN HOME NURSING Inc. is a pioneering home healthcare organization established in 2018. It was co-founded by the TRIO (Chiu-Jung Wang, Ben Kao, and Scott Chang) and powered by Buurtzorg Asia led by Dr. Stephan Dyckerhoff.
As the tenth international partner of the Buurtzorg oganization from the Netherlands, TAIWAN HOME NURSING Inc. not only practiced the Dutch Buurtzorg’s onion model of care, but also transformed it into a localized model that is suitable for Taiwanese with an upgraded home nursing IT system, interdisciplinary and integrated professionals , and an innovative organizational design focusing on empowered regional teams.
